Welcome to the Leeds West Clinical Commissioning Group Portal
Leeds West Clinical Commissioning Group (CCG) (formerly known as H3Plus) is a group of 38 GP practices based in the outer and inner north west, west and south areas of Leeds covering approximately 357,000 patients.
From April 2013, subject to the Health and Social Care Bill being passed by parliament, clinical commissioning groups will replace primary care trusts and take over responsibility for designing and commissioning (or buying) local health services. Clinical commissioning will ensure that GPs and other health professionals will have a greater say in how NHS resources are used.
Leeds West CCG is one of three CCGs in Leeds and we are all working very closely with NHS Airedale, Bradford and Leeds, the local primary care trust (PCT), to carefully manage the transfer of responsibility from the PCT to CCGs and the continuity of local health services.
We will work with patients, healthcare professionals and in partnership with local communities and local authorities to shape local services and make the best possible use of our resources.
